REWARDS PROGRAM TERMS AND CONDITIONS
1. EōS Fitness may, at its sole discretion, offer the EōS Fitness Perks program (the “Program”) according to which members may be awarded reward points through the use of the Rewards website, and certain other activities that EōS Fitness designates from time to time.
2. Opting in to email communication from EōS Fitness allows Perkville & S1P to maintain the integrity of your rewards account. You may opt out of receiving emails from EōS Fitness at any time you wish. But please remember, once you opt out, you will also not receive email updates on points.
3. To be eligible to earn Rewards Points, you must be an active member of EōS Fitness, register at Join EōS Fitness Perks and agree to the terms and conditions. You must register using the same email address you have on file for your EōS Fitness Gym membership.
4. If you elect to terminate your Membership with EōS Fitness, all remaining points in your Rewards account will expire upon your cancellation date.
5. EōS Fitness may, without notice, change the Program rules, benefits, conditions of participation, the goods or services offered in the redemption of reward points, the number of points required to redeem goods and services, make no future awards and void all prior points awarded, for any participant or all participants in the program, at its sole discretion, at any time. These changes may be made even though such changes may affect the value of the reward points already accumulated.
6. EōS Fitness has the right to make changes to the Program at any time, without notice.
7. All reward points must be earned and used according to the terms and conditions of the Program. Reward points may only be exchanged for such goods and services from clubs participating in the EōS Fitness Perks program. Reward points may not be exchanged for cash. EōS Fitness assumes no liability for any prior offers that may have been presented, but which are not a current offer at the time of redemption.
8. Retroactive credit will be given for activities undertaken before enrollment at the sole discretion of EōS Fitness.
9. EōS Fitness makes no warranties or representations, either expressed or implied, and expressly disclaims any liability (including consequential damages) with respect to the Program or any goods or services provided through the Program.
10. By opting into the Program, you are affirmatively consenting to the Program’s terms and conditions.
11. EōS Fitness agrees not to sell any of your personal information that is collected according to the Program to third parties.
12. All interpretations of these rules, terms, and conditions shall be at the sole discretion of EōS Fitness.
13. The EōS Fitness Perks program is currently available only at the following select locations: Anaheim Town Square, Anaheim/Garden Grove, Arcadia (Santa Anita), Beverly Center, Cerritos, Fullerton, Glendale, Goleta, Hollywood, La Mirada, Long Beach, Los Angeles (Downtown), Los Angeles (Downtown), Montclair, North Hollywood, Northridge, Oxnard, Rancho Cucamonga at Victoria, Gardens, Santa Barbara (Downtown), Santa Barbara (Uptown), Simi Valley, Thousand Oaks, Valencia and West Covina.
